During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Filling out your paperwork in advance ensures your time with us is well spent.
To expedite the registration process, please bring the following:
- Insurance card
- Driver's license or any form of ID
- Any medical or surgical history
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Recent test results, x-rays, or relevant records
